Laboratory Service Order Form (LSO) Instructions
To ensure a smooth and easy transition of your sample material from arrival to final reporting, it is important that the information you provide is thorough and accurate.
The form is configured so that you can easily tab through each of the 8 sections.
The Analytical Contact is the person who is contacted regarding technical aspects of the analysis and will be the recipient of results. Please ensure that the email address and phone number are correct.
Accounts Payable Contact
Any questions regarding credit card payment, purchase orders or billing is directed to the Accounts Payable contact provided. This person will also receive the final invoice.
Purchase Order or Credit Card Information
This section must be completed with either a purchase order (PO) or credit card number. Alternatively, credit card information can be provided by phone. Please note that orders will not be processed until this information is complete.
Reference PTL Quote #
If you received a formal price quotation from our sales department, a PTL quote number will be listed on the quotation.

Material Name and Sample ID
IMPORTANT: Material name and sample IDs will be reported on your final report exactly as specified. Please ensure that the spelling and identification numbers are correct.
